A graft is a unit of transplanted tissue that typically contains one to several hairs (follicular units), so grafts per cm² differs from hairs per cm²; the term is used to quantify transplant density for planning and documenting procedures.

Clinically, grafts per cm² guides treatment planning, session size and donor allocation because higher graft density generally produces fuller appearance but requires more donor hair and careful technique.
