
Open Roof Deformity
Open Roof Deformity is a structural irregularity of the nasal dorsum that can arise after hump reduction in rhinoplasty when the paired nasal bones are not closed together, leaving a visible gap or flattened area along the bridge. Patients may notice a broad, flattened, or step-off appearance across the nasal bridge, breathing is usually unaffected, but the cosmetic contour is compromised and asymmetry or visible edges may develop.
Treatment is most commonly surgical revision: controlled lateral and medial osteotomies to mobilise and reposition the nasal bones, sometimes combined with cartilage or bone grafts to rebuild contour and smooth transitions. In selected minor cases temporary nonsurgical camouflage with dermal fillers may reduce the visual defect, but definitive correction generally requires a specialist rhinoplasty revision performed once healing from the original operation is complete.
