
Symmastia (Uniboob)
Symmastia (Uniboob) occurs when the normal separation between the breasts is lost, creating one continuous breast mound across the chest rather than two distinct breasts. It most commonly follows breast augmentation and can result from over-dissection of the medial implant pocket, overly large or malpositioned implants, subglandular implant placement, thin soft tissue coverage, or changes in healing that allow implants or tissue to shift toward the midline.
Diagnosis is usually clinical and may be supported by imaging; mild cases can sometimes be managed with compression or supportive garments, but many people require revision surgery to restore the medial pocket and recreate cleavage. Surgical options include capsulorrhaphy (tightening the capsule), repositioning implants to a different plane, exchanging for smaller implants, using grafts or meshes to reinforce the midline, or fat grafting to improve soft-tissue coverage; recovery and outcomes depend on the chosen repair technique and individual healing.
