What is DHT?

DHT can be simply defined as a byproduct of the male hormone testosterone. When a particular enzyme, namely 5α-Reductase enzyme, gets into a reaction with the testosterone hormone, it is converted to Dihydrotestosterone. If the body produces extensive amount of DHT, then hair loss is experienced.

Donor area is a factor that is as crucial as the recipient area. Donor area contains the only hair type that is suitable to be used for hair transplant due to their immunity to DHT.

However, the donor area has its own limits. As hair transplantation is a method of hair replacement, it is not appropriate to extract as many grafts as needed without any concern. What are the Different Techniques Used for a Hair Transplant?

During this procedure, firstly the patient’s scalp is anaesthetised.

Then, the doctor removes a strip of tissue from the donor area.

Strip method requires an extensive medical procedure. Literally, a strip of skin is cut away from the donor area. As a result, the hair transplant patients have to carry the traces of their operation on the back of their head.

Afterwards, the surgeon and medical team cut the strip tissue into many individual follicular units via microscopic dissection techniques.

During the stereo-microscopic dissection of grafts, follicular units are isolated from the donor strip. In this way, the units are neither broken, nor damaged. To maximize growth, the whole of a single unit is kept intact. Intact follicular units provide the most fullness for the hair restoration.

In the meantime, these follicular unit grafts are being prepared, the surgeon opens tiny holes in the scalp. After that, grafts are placed in the recipient site.

What is Follicular Unit Extraction (FUE)?

Follicular Unit Extraction (FUE) uses a similar methodology to FUT except for a very important difference: In FUE, the follicular unit grafts are extracted directly from the donor area with slit method. Basically, FUE is a harvesting method.

It means that the surgeon does not cut out a strip of tissue from the donor area. Instead, the instrument is used for the separation of a follicular unit from the surrounding tissue. Here, surgeon makes a small and circular incision around a follicular unit. Then, this unit is directly extracted by pulling from the scalp, which leaves a small open hole.

This process is repeated by the hair transplant surgeon until an enough number of grafts is harvested. The duration of the process differs according to the condition of the patient, and the number of grafts. It usually takes 6 to 8 hours per operation.

It should be noted that tiny recipient sites are made in the balding area of the scalp before harvesting the grafts. For this procedure, a fine needle-point instrument is used. After that, follicular units are transplanted into the recipient site. In the end, these units will grow into healthily hair producing follicles.

After the FUE operation, it takes 7 to 10 days for donor wounds to heal. These wounds are so tiny. They are approximately 1 mm in size. They leave tiny white scars which are hidden in the hair inside sides and back of the scalp.

As aforementioned, FUT and FUE have essentially the same process of creating recipient sites, and placement of follicular unit grafts. The main difference is that FUE offers better quality and quantity of the necessary graft numbers. You should also remember, in the FUT technique, the operation leaves an unpleasant scar on the donor area.

What is Direct Hair Implantation (DHI) / Unshaven Hair Transplant?

DHI (Direct Hair Implantation), also known as Unshaved Hair Transplant, is done by a special Choi implanter pen. It is a minimally invasive procedure. Hair transplant candidates with partial baldness are ideal candidates for this type of procedure

The difference DHI method provides is that it allows a hair transplant operation without shaving the hair. Hair transplant patients can go on with their lives without the traces of operation.

In Unshaven Hair Transplant, a different tool, named Choi pen, is used after the regular extraction with slit method. After the grafts are extracted, they are loaded into the Choi pen. Choi pen has mechanically designed in a way that allows to combine the last two stages of hair transplant, channel opening and graft implantation. After the extraction, channel opening and hair transplantation can be done at the same time.

Candidates with traction alopecia, receding hairlines, or partial baldness can have DHI treatment. It usually takes 4 to 8 hours.

The recovery time for DHI hair transplant is 2 to 3 days.

Direct Hair Implantation (DHI) has one disadvantage. It only allows up to 2500 grafts to be transplanted per session. Therefore, hair transplant patients may need multiple sessions if the balding part requires more than 2500 grafts.
